Mail the following documents, along with a postage-paid, self-addressed envelope, to the Treasurer - Town of Sumpter Treasurer, PO Box 365, Baraboo, WI 53913:
- Check made out to the Town of Sumpter for the appropriate amount from the Town of Sumpter Fee Schedule.
- A copy of the statement from your vet showing the name, breed, and color of the dog.
- Proof of rabies that includes the following information:
- Name/address of the veterinary that provided the vaccination
- Date of the shot
- Expiration date of the shot
- Name of the vaccine
- Serial number or lot number
